Eligibility Requirements

You may qualify for PACE services if, at the time of enrollment, you:

• Are age 55 or older
• Live in one of the service area zip codes
• Are certified by the state as eligible for nursing facility level of care
• Are able to live safely at home with services provided by Saint Joseph PACE

A PACE enrollee may be, but is not required to be, any or all of the following:

• Entitled to Part A
• Enrolled under Part B
• Eligible for Medicaid

Saint Joseph PACE will not restrict enrollment based on Medicare or Medicaid eligibility.

PACE services are typically free to seniors who qualify for Medicare and Medicaid or Medicaid only. Participants who don't qualify for Medicaid will be charged a monthly fee and have the option to privately pay. To learn more about eligibility, please contact us.
